You will need to have the following two documents ready to upload before starting the application. Each document must be uploaded individually. To prepare your documents, scan or photograph each document separately and keep the images on your desktop or mobile device for easy upload.
IDENTIFICATION & RN LICENSE
You will need:
1) A non-expired, non-temporary copy of your government-issued photo ID (photo side only)
2) A copy of your RN license showing date of expiration or screenshot of RN License Look up
*SUPERVISOR INFORMATION
You will also be required to provide your supervisor's:
- Name (First, Last)
- Title
- Work Facility Name

-State
-Work Email
-Work Phone
All applications my be reviewed and approved by C-NET before a testing permit can be issued.
